By registering under the PhilHealth Konsulta Program through Shalom Dynamic Health Care Co.’s online platform, I voluntarily agree to participate in the program. I consent to the collection, processing, and secure storage of my personal information in compliance with the Data Privacy Act of 2012 (RA 10173). I authorize the use and sharing of my data with PhilHealth for eligibility verification, benefit processing, and health service provision. I understand that my registration covers me under the program for one (1) year, during which I cannot transfer to another Konsulta facility. After this period, I may request a transfer by submitting a written notice. By proceeding, I confirm that the information provided is accurate and true. Please note that registration on this platform is subject to verification. You will be notified once your registration has been successfully verified.
